Six numbers from your own PBJ data return your exact staffing score out of 380. Not an estimate — CMS publishes the point range for every measure, so this is the same arithmetic they run.
Case-mix adjusted RN hours per resident day, all days.
RN + LPN/LVN + aide hours per resident day, case-mix adjusted.
Case-mix adjusted total nurse HPRD across Saturdays and Sundays only.
Percentage of RNs who left over the 12-month measurement window.
Percentage of all nursing staff who left over the 12-month window. Includes agency and contract clinicians who worked 120+ hours in a 90-day period.
Number of nursing home administrators who left during the measurement period.
Ranked by what each measure is worth against how much it has to move — with the ones that can respond in a single quarter first.
Points are only half of it. A staffing rating is assigned outright at one star — regardless of these six measures — if you report four or more days in a quarter with no RN hours while residents are in the building, miss a submission deadline, or fail a staffing audit. Clear those before optimising anything.
Go deeper: The Cheapest Star on the Board walks the full playbook, and how CMS calculates nursing home turnover explains why your agency clinicians count toward the stability measures.
Every point band on this page is published by CMS in the Design for Care Compare Nursing Home Five-Star Quality Rating System: Technical Users' Guide (July 2026 edition) — Table A2 for the measure ranges, Table 3 for the star cut points. Nothing here is modeled or estimated.
The thresholds are fixed published values rather than a live ranking against other facilities, so improving your own numbers earns the points regardless of what anyone else does. They change only when CMS republishes them. Older copies of CMS methodology documents circulate widely, including on cms.gov — check the cover date on anything you rely on.
