Where these numbers come from

Every point band on this page is published by CMS in the Design for Care Compare Nursing Home Five-Star Quality Rating System: Technical Users' Guide (July 2026 edition) — Table A2 for the measure ranges, Table 3 for the star cut points. Nothing here is modeled or estimated.

The thresholds are fixed published values rather than a live ranking against other facilities, so improving your own numbers earns the points regardless of what anyone else does. They change only when CMS republishes them. Older copies of CMS methodology documents circulate widely, including on cms.gov — check the cover date on anything you rely on.
